Where can I find good, positive examples/template of filling in a uk police related application form?

I am looking for a site on the web which shows good examples/templates of what to say and how to put it down on paper (application form) as I am trying to fill in a police related application. Earlier efforts of mine tend to whaffle on - I need to make my applications strong, appealing and to the point. Any one out there can help?
